INITIAL VISIT

First visits at our office are also referred to as “consultations”. Some children only require 1 visit to a pediatrician; however, many others continue to access care long-term in our office with the support of their family physician for overall health care needs.

If you are preparing for your child’s first visit, please bring the following items:

  • Alberta Health Insurance card(s)
  • Parent’s photo ID
  • Complete immunization records
  • List all of your child’s medications and dosage (or take pictures of the labels)
  • Medical records from your previous pediatrician (If you do not have them, we can request them after you have signed a release of records form)
  • Completed forms (please print this packet, complete the forms, and bring them with you to your first appointment)

MEDICAL

If your child is being seen for a medical concern and has been seen by a previous provider, it would be helpful to have medical records from your previous pediatrician (if you do not have them, we can request them after you have signed a release of records form). Our pediatricians are able to access previous investigations and most specialist consultations via Alberta Netcare.

ADHD AND OTHER SCHOOL-AGE LEARNING, BEHAVIOR, AND/OR EMOTIONAL CHALLENGES

Prior to booking your first visit for a school or behavior related problem we require some forms be completed by parents and teachers and by adolescent patients. Please download these questionnaires and provide the teacher questionnaires to the teacher(s).

Step 3 – Return Questionnaires Completed by Parent, Adolescent, and Teacher

All of the needed questionnaires must be returned completed before an appointment will be booked. Documents can be uploaded, mailed, faxed or e-mailed (please contact clinic for further details). Please include copies of any other relevant reports (e.g. psychology, speech language pathology, occupational therapy, report cards, etc.)
